Service Attendance Guidelines

Dear Attendee,

We are grateful to finally be coming back together as a community. As we gradually re-open our doors, your health and safety are our highest priority. We have gone to great lengths to ensure that our Shul is a clean and safe environment. By following the policies below, you can help us to keep it that way. Signing this form acknowledges that you will adhere to these guidelines. You will be asked to submit this signed form prior to your arrival at the Shul. If you have not read and signed this form, you will not be permitted to enter the Shul. All children under age 18 in attendance must be listed below, and accompanied by a responsible supervising adult who has read, signed and submitted this form before their entry.

  1. If you are sick, please stay home.
  2. Attendees will be screened at the door by a volunteer for fever, chills, cough, shortness of breath, GI illness, and loss of taste/smell. If any screening questions are answered yes, the attendee and those arriving with them will be asked to return home.
  3. Attendees are required to bring and wear a face covering at all times. They must be worn above your nose, over your mouth and under your chin. Disposable masks will be provided if you do not have one.
  4.  Avoid touching your eyes, nose, or mouth during the service.
  5. Hand sanitizer will be available for use when entering and exiting the sanctuary.
  6. Only one person at a time allowed in the rest rooms. There are restrooms opposite the office and in the rear hallway near the classrooms.
  7. Please bring your own Kippot and Tallit. If not, Kippot are available for use, but we ask that you please not return them. Once you take it, it is yours.
  8. Even while wearing a mask, please cover your cough or sneeze with your elbow.
