In Person Service Attendance

We are grateful to finally be coming back together as a community. As we gradually re-open our doors, your health and safety are our highest priority. We have gone to great lengths to ensure that our Shul is a clean and safe environment. For the health and safety of our community, a self-declaration of health form is required of all attendees for each service they sign up for.  Each service is permitted up to 25 attendees   All attendees will also be asked to a read and consent to our Service Attendance Guidelines.

If you do not timely submit a signed form you will not be permitted inside the Shul.   One form must be completed for every person attending each service.  All children under age 18 in attendance must be accompanied by a responsible supervising adult who has read, signed, and submitted this form before their entry. Signing this form acknowledges that you will adhere to these guidelines.
